Moreover, any delays in diagnosis may adversely influence the prognosis, thus rapid imaging or investigations should be prioritized [86]. Based on a meta-analysis of studies performed between 1970 and 2009, Johnson et al. concluded that a single negative whole-leg compression ultrasonography (CUS) excludes both proximal and distal DVT, and that repeat CUS is not required to rule out DVT [87]. Per a study done by Geersing et al., a low Wells score paired with a negative D-dimer test can rule out DVT. Whereas for those with suspected recurrent DVTs, adding one point to the rule allows for a safe DVT exclusion [88]. Lately, two-point and three-point point-of-care ultrasound (POCUS) procedures have come up as excellent tools for diagnosing DVT in the ED, displaying high specificity and sensitivity, especially when administered by a POCUS-trained emergency physician [89].

However, the relative failure rate of a single restricted CUS was unclear, making it only suitable for low-risk applications [92]. The onset of drug effects is about 15–30 seconds for the intravenous route and 3–5 minutes for the intramuscular or subcutaneous route.

The search for a rapid increase in the concentration of drugs in the blood is the primary cause of intravenous drug abuse. Furthermore, many users crush tablets of one or more drugs for use as an injectable solution, and when doing so, they also inject the drug’s adjuvants, which carry health risks independent of the ones from the active ingredient from which they seek pleasure [24]. Opioids are the most used intravenous drugs; in fact, heroin is the most prevalent drug among intravenous drug users [25]. In this paper, we describe the epidemiology, presentation, diagnosis and management of infections in IDUs who may present to acute physicians and the management of drug dependency on acute medical wards. Although syringe exchange programs in other locations abroad are still in the preliminary stages of development, none so far has reported any increase in drug use among clients.

As noted earlier, the first ex-addict outreach program in New Jersey evolved from one that taught sterilization methods into one with expanded treatment capacity (Jackson and Rotkiewicz, 1987; Jackson and Baxter, 1988). The outreach programs in New York and San Francisco that distribute bleach have had to develop referral-to-treatment programs and street counseling components to keep up with the demand for these services (Des Jarlais, 1987b). For these women, asking a man to use a condom provokes the fear of breaching relations that may fulfill the woman’s sexual, personal, financial, and drug needs. Illicit drug use has been a long-standing social problem in this country, and public policies to deal with it have resulted in drug treatment and law enforcement programs. People who inject IV drugs are at risk for many illnesses, the most common being hepatitis and HIV. Each time someone injects a drug, they increase their risk of contracting infections and developing sepsis, whether they use these drugs occasionally or on a regular basis. HIV is only normally transmitted through anal or vaginal intercourse or by contaminated medical equipment. While medical equipment used in clinical settings normally goes through rigorous sterilization processes to prevent the spread of disease, medical equipment used outside of the health care system can easily spread infection. Sharing a needle that has been used by another person can create the risk of catching bloodborne illnesses like HIV.
